ACCOMMODATION AGREEMENT
I agree that Maryvale shall not be liable for any damage or injury to persons or property arising out of the use of the premises. In the event of a dispute, I waive my right to a jury trial, and I hold Maryvale harmless from all liability and loss on account of any such damage or injury, except in the case of gross-negligence on the part of Maryvale.

No emotional support animals or pets are allowed in the buildings. Service dogs are allowed with proper documentation.
Maryvale does not allow cooking in guest rooms. A hospitality kitchenette is available for preparing simple meals and a kitchen may be available for rent.

Smoking is not allowed on the premises or outside within 20 feet of entrances. I understand that I will be charged $200 per violation.

Alcoholic beverages are permitted only with prior authorization. I must contact the Director of Hospitality if I wish to obtain permission. I understand that I will be charged $200 per violation.
I will need to reimburse Maryvale for any damage to property or equipment during my stay which I cause by my own willful or negligent actions. I am not responsible for breakage caused while correctly using equipment.
I assume full responsibility for my own well being. I am responsible for conveying any food allergies or other limitations to Maryvale. Maryvale does not have allergen-free dedicated food preparation spaces so dietary cross contamination can occur. I acknowledge that depending upon the type and severity of my food allergies, that Maryvale might not be able to accommodate my needs. I am responsible for following all signage and using good sense. There is inherent risk in certain activities which I may engage in at Maryvale (such as: walking the grounds, hiking, using kitchen equipment, using exercise equipment), and that I am responsible for choosing activities which are safe for me.

PAYMENT
The fee for this retreat is $250 (per person) which includes a non-refundable deposit of $75 must be paid prior to attending the retreat. We are unable to reserve a room until the registration fee has been paid.
You may pay online with a credit card, by Venmo, mail us a check, or call us at (701)845-2864 to make other arrangements. If you pay by check, the check must be received three business days before the start of the retreat.
