I accept responsibility for the actions and wellbeing of the individuals attending this event (hereafter "the Attendees"). If I wish, I may require the Attendees to sign copies of Maryvale's "Individual Accommodation Agreement," and so each individual who signs would be responsible for their own actions and wellbeing. I understand that Maryvale will provide these forms at my request, and that Maryvale is not required to offer them to me.

I agree that Maryvale shall not be liable for any damage or injury to persons or property arising out of the use of the premises by the Attendees, and I hold Maryvale harmless from all liability and loss on account of any such damage or injury.

I assume all responsibility for the health and well being of the Attendees. I am responsible for conveying any food allergies or other limitations of the Attendees to Maryvale. Maryvale does not have allergen-free dedicated food preparation spaces so dietary cross contamination can occur. I acknowledge that depending upon the type and severity of Attendees food allergies, that Maryvale might not be able to accommodate their needs. I am responsible for ensuring that Attendees follow all signage and use good sense. I acknowledge that there is inherent risk in certain activities which the Attendees may engage in at Maryvale (including but not limited to: hiking, using exercise equipment, using kitchen equipment) and I take full responsibility for the choice of activities of the Attendees.

I fully understand that I shall be held accountable for any damage to property or equipment by the Attendees during our stay at Maryvale. I understand that all Maryvale policies and procedures will be adhered to strictly.  I understand that Maryvale operates under the directives of the Catholic Diocese of Fargo and that Maryvale reserves the right to determine appropriate use of its facilities, and I agree that the Attendees' use of the facility will not be for purposes that stand in opposition to Maryvale's mission nor the teachings of the Catholic Church.

▪ No emotional support animals or pets are allowed in the buildings - service dogs are allowed with proper documentation. 
   
▪ Maryvale does not allow cooking of any kind in guest rooms. 
   
▪ Smoking is not allowed on the premises or outside within 20 feet of entrances. If any attendees violate this policy, I will be charged $200 per violation. 
   
▪ Alcoholic beverages are permitted only with prior authorization. I must contact the Operations Manager if I wish to obtain permission. If any attendees violate this policy, I will be charged $200 per violation. 

By selecting "I Agree" below, I am waiving my right to jury trial and agreeing that I am personally responsible for all Attendees who have not signed Maryvale's "Accommodation Agreement."
